PROGRAM DESCRIPTION:
The primary goal of Re-Entry House is to foster personal growth
in a continuum of care that supports an individual's program of
recovery from chemical dependency. This is accomplished with the
guidance and support of a community of peers, each contributing
to the personal growth of self and others.
CRITERIA
FOR ADMISSION:
Gaudenzia
Re-Entry House is a state licensed halfway house serving males and
females over the age of 18 years old. The length of stay is variable,
but no less than 90 days.
Prospective residents must meet the following criteria to quality
for admission.
Successful completion of a treatment program licensed by the Pennsylvania
Department of Health Office of Drug and Alcohol (BDAP), prior to
admission. Be free of all controlled substances pursuant to a urinalysis.
Possess a Social Security card and birth certificate.
Possess an ACCESS card or other form of medical insurance. Present
no medical condition which requires 24 hour medical monitoring.
Exhibit motivation to continue in the recovery process.
ADMISSION
/ INTAKE:
All prospective residents will be interviewed to establish their
level of readiness for this treatment modality.
Orientation
Phase (Up to 10 days)
During this initial phase, the client will be oriented to the rules
and regulations of the house. Data is collected from the client,
as well as external sources to develop the biopsychosocial profile
and to facilitate the formulation of an initial treatment plan.
Level
1 (30 days)
This phase of clinical treatment builds upon the continuing development
of the biopsychosocial profile, and addresses the emotional, spiritual
and behavioral needs of the individual. Clients are encouraged to
identify and express emotions, develop positive attitudes and participate
in 12 step programs.
Level
II (30 - 60 days)
The treatment plan is changed and modified throughout the treatment
process. During this phase, emphasis is placed on the development
of a concrete and realistic aftercare plan. Contact with an appropriate
outpatient program is initiated, and a linkage with community resources
is formed to facilitate the transition from re-entry to discharge.
Level
III (30 - 90 days)
During the final phase of treatment, emphasis is placed on the client's
ability to make appropriate and responsible decisions, while increasing
exposure to the community. Clients at this level will be enrolled
in an educational or job training program, or will be gainfully
employed. Individuals will learn to budget their income, and to
save money, which will enable them to live independently. Attention
focuses on the successful integration of the values of a drug-free
lifestyle as the client prepares for the discharge process.
CRITERIA FOR COMPLETION:
A client is eligible for completion of treatment and discharge when
the following criteria has been met:
