LENGTH OF TREATMENT: Variable

PROGRAM CAPACITY: 30 adolescents

CRITERIA FOR ADMISSION:
• Male must be 13 – 18 years of age
• Client must have an evaluation by a drug and alcohol outpatient services provider utilizing assessment protocol approved by public and private funding sources and client meets level of care recommendation.
• Detoxification can be Arranged if Necessary
• General Medical Stability
• No risk of harm to self or others

HELPFUL AT ADMISSION:
• Current Form of Identification
• Insurance or Medical Assistance Cards(s)
• Social Security Card
• Prescribed Medications
• Immunization and Dental Records
