Records Requests
Any requests for records are facilitated through Gaudenzia’s Compliance Department.
For all medical record-related requests, please send HIPAA and 42 CFR Part 2 compliant release forms to Compliance@gaudenzia.org or by fax at 267-443-5912.
Please be advised that Gaudenzia and its respective programs are federally assisted substance use disorder treatment facilities, and as such are governed by the federal regulations held under HIPAA (Health Insurance Portability and Accountability Act) and 42 CFR Part 2 (42 U.S.C. 290dd-2), which regulates the confidentiality of substance use disorder patient records. Gaudenzia and its respective programs must adhere to the confidentiality regulations held by 42 CFR Part 2, with these rules, standard HIPAA-compliant release forms are not sufficient for the release of substance use disorder-related records.
